A bill passed last week in the Maryland legislature should be a huge benefit to families affected by autism. The bill sets up a pilot program in the State Department of Education to teach health-care providers how to detect the earliest signs of the neurological disorder in children under 2 years of age. Diagnosis at such a young age by pediatricians would enable both professionals and family members to begin earlier intervention, providing autistic children a better chance at a normal life.

Early detection is especially important with autism, because research and experience indicate that the earlier the disorder is detected, the better the response to treatment and therapy.

The legislation unfortunately does not provide funding for the program, which would not only set up a program to work with some 30 pediatricians and initially set up two screening sites in Maryland, but also charge the state health department with studying how well participating doctors monitor their young patients. It is estimated that initial costs could be around $250,000.

Autism is characterized by unusual brain development, but precisely what causes it is not clear to researchers. The disorder usually becomes evident at around age 3, when such problems as delayed communication abilities, stunted social development and sometimes odd patterns of movement are noted.

But researchers believe some of the indications can be detected between 18 months and 2 years of age, and early screening and diagnosis gives parents and professional health-care providers extra time to work with these children while their brains are still developing rapidly, perhaps by establishing compensation in different regions of the brain.

The three-year pilot program approved by both the Maryland House of Delegates and Senate last week would require that screening practices used in pediatric health-care settings be assessed by the state Department of Health and the Department of Education. Special training will be provided to some 30 selected pediatricians around the state to help them recognize the earliest signs of autism, who would notify parents and also make them aware of available intervention services.

The pilot program would also establish and incorporate model screening practices as part of well-baby visits for 1- to 3-year-olds in two jurisdictions in the state.

The cost of the program should not be a deterrent, because money spent now to establish early intervention could reduce the cost of special education and rehabilitative services provided by the state to children who are diagnosed at older ages.

Autism is now believed to affect as many as one of every 166 children, according to the Centers for Disease Control and Prevention. Maryland is in a position to make serious inroads against this devastating developmental disorder.

Funding is, as always, an issue -- but we believe money spent on this program would be, in the long run, well spent.
